We will, as a courtesy, process your insurance benefits in our office. We ask that you review all estimates and call your insurance company with any questions. We can only guarantee Dr. Tracy’s fees and ESTIMATE your insurance benefits.

I agree that I am fully responsible for the total payment of all procedures performed in the office All Family Dental Care, Colleen Tracy, DDS. I understand that my portion is due at time of treatment and that there is no guarantee of dental benefits should they be available. I also understand that if my dental benefit company does not pay within sixty (60) days that I am responsible for payment in full of entire treatment.

  • We require 2 business days advance notice to change or cancel an appointment. There maybe a $100 per hour charge made for late cancellations or missed appointments.
